
Patient Records, Licensing & Complaints
Clear Lake Integrative Medicine is committed to protecting your health information and making it easy to understand your rights as a patient. This notice explains how to request your records and how to contact the appropriate Texas licensing authority.
In accordance with Texas Health & Safety Code §181.105 (House Bill 4224), the following information explains how to request your health care records from Clear Lake Integrative Medicine, contact the appropriate Texas licensing or disciplinary authority, and file a consumer complaint regarding your health care, health records, or protected health information.
1. How to Request Your Health Care Records
You have the right to request access to or copies of health records maintained by Clear Lake Integrative Medicine.
Submit Your Request
Please submit a written request for your records by email, mail, or in person. If you call us, our staff can assist you with the records-request process or provide any authorization form that may be required.
Tell Us What Records You Need
Please include your full legal name, date of birth, approximate dates of treatment, the specific records you are requesting, where or to whom they should be sent, and your preferred delivery method. Please do not send Social Security numbers, financial account numbers, or other unnecessary sensitive information by unsecured email.
Identity Verification
To protect your privacy, we may ask you to verify your identity or authority before releasing protected health information. If someone is requesting records on your behalf, appropriate written authorization or documentation of legal authority may be required.
Processing Your Request
When Texas Health & Safety Code §181.102 applies, an electronic health record requested in writing will generally be provided no later than the 15th business day after the request is received, subject to applicable exceptions. If a fee permitted by law applies, we will inform you.

2. Contact the Appropriate Licensing Authority
Because Clear Lake Integrative Medicine provides care through licensed professionals from more than one health care discipline, the appropriate state licensing board depends on the type of provider involved. You do not need our permission to file a complaint.
Texas Board of Chiropractic Examiners (TBCE)
Texas Board of Nursing (BON)
Texas Medical Board (TMB)
When filing a complaint, provide as much information as possible, including the name of the provider involved, the dates of the incident, the treatment or services involved, and a detailed description of your concern. The Texas Medical Board does not regulate nurses acting within the scope of nursing practice.
3. Texas Medical Records Privacy & Consumer Complaints
Texas Health & Safety Code §181.103 requires the Texas Attorney General to maintain consumer information about privacy rights for protected health information. A consumer may also submit a complaint through the Texas Attorney General's Consumer Protection Division. For concerns that personal or health information has been unlawfully collected, shared, disclosed, or mishandled, select the Privacy complaint process. Do not include unnecessary confidential information such as Social Security or financial account numbers.
